Change in Major or Certificate Program - Graduate Counseling Students
Use this form to add and/or drop majors and minors.
Use this form to change courses or sections of your registration. For a section change, complete the form and return to the Registrar’s Office by the appropriate date. For a course change, complete the form, have your advisor sign off on the change, and return to the Registrar’s Office by the appropriate date. If a class is closed or if you want to register for a class for which you have not met all prerequisite requirements, you must have written approval from the division chair.
Up until the tenth week of the semester (or same percent [5/7] of partial semester classes), students may withdraw from a course with a grade of "W." This should be done only after consultation with academic advisors, the Registrar's Office, and the Financial Aid Office, as a reduction of course load below 12 credits may affect eligibilities for athletics, scholarships, or financial aid. After the tenth week of the semester, no change in registration is permitted except for total withdrawal from the university.

Application for graduation must be made by September 30 for the following April and August ceremonies and by January 30 for the following December ceremonies. The application form must be completed and signed by all academic advisors before returning to the Registrar's Office for processing.
Transfer Course Evaluation
Use this form to inquire about courses from other colleges/universities transferring in to Indiana Wesleyan University. If you are requesting that the course meet a requirement for your major you must obtain the signature of your Division Chair. This form must be completed and turned in to the Registrar's Office for processing. It is recommended that this form is processed prior to registering for courses at another college/university.

Leave of Absence Petition
Full-time degree seeking students may use this form to request a Leave of Absence (LOA) in certain circumstances to preserve their current catalog rights. The student (top) portion of this form must be completed and turned in to the Registrar's Office for processing.

Final Exam Change Request
If you have more than two final exams on one given day, arrangements can be made with your professor to change the exam that falls in the middle. It is your responsibility to complete this form and provide the requested information to the professor. This one exam can then be rescheduled.
Intent to Depart
Complete this form if you do not intend to return to IWU the following semester. This form is not to be used for withdrawing from the current term.
